OT Evaluations

Formal Occupational therapy evaluations are offered when an individual is experiencing difficulties in their daily life due to physical, mental, emotional, psychological, and/or developmental challenges. An OT evaluation, dives into an individuals strengths, challenges, and areas of deficits to develop a holistic plan of care for treatment. OT Evaluations are used to determine eligibility for services. At Empowered Minds, we offer a variety of standardized and non standardized assessments, including, but not limited to the following:

  • Evaluation Tool of Children’s Handwriting

  • Bruininks-Oseretsky Test of Motor Proficiency (BOT-3)

  • Assessment of Motor & Process Skills (AMPS)

  • Vineland Adaptive Behavior Scales

  • Occupational Interview

  • Motivational Interview

  • Family/Caregiver interviews

All OT evaluations are completed at the pace of the client with accommodations and modifications as needed.
